The Linnaeus Medical Center offers structural care to patients with diabetes mellitus (sugar disease), cardiovascular diseases or COPD (lung diseases).
Diabetes mellitus:
The Linnaeus Medical Center has a specialized and structured program for the guidance of patients with diabetes mellitus. Diabetes mellitus is also called diabetes.
Checks and guidance are largely done by the Practice Support General Practitioner - Somatics (POH-S). You can be referred to the POH-S consultation hour by your general practitioner.
With regard to the structural care provision concerning diabetes, the practice works together with the ROHA, the OLVG and various laboratories. In addition, all practice employees regularly follow further training in this area.
Cardiovascular diseases:
The Linnaeus Medical Center has a specialized and structured program for the guidance of patients with cardiovascular diseases or with an increased risk of these. This is also called Cardio Vascular Risk Management (CVRM).
During check-ups and guidance, a distinction is made between two groups of patients, namely:
- People who have had a cardiovascular disease (heart attack, CVA) and for whom it is very important that the risk of recurrence remains as small as possible.
- People with an increased risk of developing cardiovascular diseases, for example due to high blood pressure, high cholesterol and/or smoking.
This risk should be kept as small as possible through advice and medication.
For all guidance, information, insight and a healthy lifestyle are very important.
Checks and guidance are largely done by the Practice Support GP - Somatics (POH-S). You can be referred to the POH-S consultation hour by your GP.
COPD:
The Linnaeus Medical Center has a specialized and structured program for the guidance of patients with COPD.
Patients are invited annually to discuss how things are going and whether the medication is still correct. Spirometry is performed once every few years, depending on your complaints.
