General practitioner care is a dynamic area that is constantly subject to change. In order to guarantee the quality, accessibility and continuity of care, to improve it where possible and to align it as well as possible with current patient situations, the Linnaeus Medical Centre maintains various partnerships. Some partnerships have a directly noticeable effect on patients, for example where it concerns observation, collaboration with pharmacies or district nursing. Others are less noticeable to patients, for example where it concerns GP consultation, further training or current social issues.
Huisartsenpraktijk Dapperbuurt:
There is an intensive collaboration with Huisartsenpraktijk Dapperbuurt (GP S. van Haeringen and GP S. de Kort). During practice closures (due to holidays, further training of employees, etc.), Huisartsenpraktijk Dapperbuurt and the Linnaeus Medical Center cover for each other. The computer system and patient data of the closed practice can be viewed and used in a secure manner by the acting GP. In addition, the practices work together in the area of further training for employees.
Coordinated care:
Structured health care for chronic conditions such as diabetes mellitus, cardiovascular diseases and lung diseases takes place in the form of chain care under the auspices of the ROHA (Regional Organization of General Practitioners Amsterdam).
Wijkgroep oosterpark /HAGRO (Huisartsengroep Amsterdam-Oost):
Linnaeus Medical Center is part of the Oosterpark District Group (Amsterdam-East General Practitioner Group) together with general practitioners de Jong-Lettink, Tabak, Sluis, De Jong-Ten Hove, Goede, Van Haeringen and De Kort are part of this group.
Pharmacy:
There is structural consultation with the Linnaeus Pharmacy, Watergraafsmeer Pharmacy, BENU Pharmacy Van Swinden and Park Pharmacy Amsterdam.
OLVG:
Monthly consultations are held with the OLVG. Various specialisms, a delegation of the medical staff and representative general practitioners are present. Further training is often followed as provided by the OLVG under the name Vice-Versa.
District Old East:
Several times a year, consultations are held with the City District (Oud-Oost) on issues at the interface of health and well-being.
Community nursing:
Consultations are held with district nursing (Buurtzorg, Evean and Cordaan) and home care organizations.
